VNA provides hospice care in the following counties:

HOUSECALLS

For some patients receiving all their health care at home is the best fit for their needs. Other patients include VNA HouseCalls as a supplement to visits from their own doctor who may not provide this service. HouseCall patients may be eligible to receive discount prescriptions written by a VNA provider at either VNA or Walgreens locations.

Through the VNA HouseCalls program, your provider will:

• visit your home to provide care and treatment for chronic or acute illness
• communicate with your regular physician about your treatment plan
• offer medication management and refills

HOME HEALTH

VNA home health offers nursing-based treatment of an illness or injury to help patients heal and regain their independence. Examples include caring for pressure sores or a surgical wound and monitoring of a serious illness. VNA has been providing care in the home since its founding in 1918.

Home care is usually more convenient, less expensive, and just as effective as the care you can get in a hospital or nursing home.

A registered nurse from VNA’s Home Health program will:

• Make regular visits to your home to provide care
• Coordinate the services you need from other healthcare providers, including doctors, therapists, social workers and nurse aides
• Keep you, your family and your doctor informed about your progress and care plan

HOSPICE

Losing a loved one or facing a terminal illness can be a long journey. VNA is here to help.

You can call the VNA Hospice team even if you or your loved one are not ready to start hospice care. Our intake nurses will answer your questions or help you find a resource that might be helpful to your family.

Topics we can help with include:

• Advance directives
• End-of-life issues
• Making plans to provide care for a loved one with a terminal illness
• Grief and loss issues
• Understanding palliative care vs. curative treatment
