1. Introduction & Background VNA Health Care (VNA) is a not-for-profit health care system and is in need of asphalt replacement for parking spaces behind the building located at 400 N. Highland Ave. Aurora, IL. The objective of this Request for Proposal is to locate a source that will provide the best overall value to VNA. While price is a significant factor, other criteria will form the basis of our award decision. The price proposal should indicate the overall fixed price for the project. VNA is tax exempt. If you have a standard set of terms and conditions, please submit them with your proposal. All terms and conditions will be subject to negotiation.

Proposals must be received prior to May 18, 2021 at 4:00 p.m. via email to abaudouin@vnahealth.com be considered. Your proposal will be acknowledged when received via email. Proposals must remain valid for a period of 45 days.

Project Description
Commercial Property
Location: VNA Health Care 400 N. Highland Ave. Aurora, IL
Behind building – next to rail road tracks – the entire length of the building.

Partial Depth
Grind and Repave
Surface grind up to 2.00” existing asphalt surface area
Grind depth not to exceed 2.00”
Dispose of asphalt spoils at approved dumpsite
Tack coat asphalt surface
Install 2.00” surface course material. No change in pavement elevation or drainage are planned.
Roll to compaction with vibratory roller
Note: Disclose if there is an additional charge for any removal or re-installation of sub-base, “binder or CA-6 Gravel, to insure stable sub-base exists prior to installation of surface layers.
Price Separately
Striping: Restripe completed in yellow water based material meeting or exceeding Illinoi State specifications M-122-96. Yellow Stripes for as many parking spots that can fit on property - striping no handicap boxes. Specify the number of parking spaces in your proposal and total square feet of the project. VNA will rate proposals based on the following factors, with cost being the most important factor: 1. Responsiveness to the requirements set forth in this Request for Proposal 2. Relevant past performance/experience 3. Cost

VNA reserves the right to award to the bidder that presents the best value to VNA as determined solely by VNA in its absolute discretion.
