VNA Health Care is the largest Community Health Center in the Chicago suburbs, and has been serving the community for over 100 years.

  • VNA currently operates 16 clinics across the Chicago suburbs and serves more than 75,000 patients each year
  • VNA is accredited by the Joint Commission as a Primary Care Medical Home and services include: Family Practice, Internal Medicine, Pediatrics, OB/GYN, Mental Health, Dental, Optometry, Podiatry
  • VNA Obstetricians and Midwifes deliver over 1500 infants per year at local hospitals
  • VNA provides comprehensive chronic disease management. Providers care for over 5,000 individuals with Diabetes and High Blood Pressure ranks as the #1 diagnosis among patients
  • VNA home-based services include medical House Calls, Home Health and Hospice
  • VNA serves individuals with or without insurance - approximately 1/3 of all patients are uninsured
  • Typically over 63% of all patients identify as Hispanic; 14% as Black/African-American; 4% as Asian
  • VNA has been recognized for its delivery of excellent patient outcomes.
  • VNA ranks in the top 3.5% in the nation among Community Health Centers for volume of patients served

When the COVID-19 pandemic began, VNA Health Care immediately adapted to care for those in need.

  • Within 7 days of the pandemic, VNA had instituted telemedicine for the communities they serve
  • VNA has maintained in-person care throughout the pandemic
  • VNA experienced a 70% increase in call volume
  • VNA has provided over 40,000 COVID tests so far
  • Now, VNA Health Care is leading the fight in the Chicago suburbs to address vaccination gaps facing communities of color.
  • VNA has administered over 60,000 COVID vaccines so far, and is providing immunizations 6 days/week at multiple VNA locations by appointment and walk-in. For more info visit:
  • VNA is collaborating with local churches, school districts, and community organizations and has added walk-in and drive-up vaccination services to vaccinate more people in the coming weeks
  • VNA has vaccinated individuals in 200 community events so far
  • VNA also administered more than 12,000 flu shots to keep people healthy during the fall and winter COVID spikes and has begun the 2021 flu shot campaign

Through the Pandemic Health Navigator Program, VNA has additional tools, resources and Community Health Workers to support the community's fight against COVID.

  • VNA expects to coordinate services for over 70,000 community members impacted by COVID
  • VNA provides services in Kane, DuPage, Will, Kendall and Grundy counties and has 48 Community Health Workers in the program
